If you are NEW to this office, be sure to select either the NEW PATIENT CONSULTATION or STUDENT CONSULTATION. For current patients, be sure to select which service combination you want as treatment times/pricing vary depending on the combination.

Please note the office will be CLOSED from Dec 31st thru Jan 4th. The office will open for normal business hours beginning Monday, Jan 5th.
